Adding Your Forms to Laser App

The cost of adding forms to Laser App is $20 per fillable page. This fee is a programming cost, and is only charged if a new form is added or a form needs to be updated. There is no maintenance or annual fees associated with having your forms on Laser App.

To begin the process, first you will need to send the form to the Forms department. We prefer forms in PDF format, however we will accept other formats (such as Microsoft Word and Excel). Please contact the Forms department if you wish to verify that a different format is acceptable. Once we receive the form, we will send you a receipt of confirmation and review the form(s).

If there are additional forms for which you will incur a cost, we will ask you to sign a purchase authorization prior to beginning work on the forms. After both Laser App and a representative from your company signs the forms, we will add the forms and send you an invoice. A 50% deposit may be required prior to forms. The programming progress typically takes 10 to 30 calendar days.

Once your forms are available in Laser App, any registered rep with your broker dealer will be able to download the forms and use them.

Updating Your Forms in Laser App

When you add your forms to Laser App, we will ask you how you would like your forms updated:

  • Website
  • Email
  • Fax
  • Phone
  • US Mail

If you select Website:
We will check your website every 30 days for form updates. If there is an update, we will contact you to seek approval for the cost to complete the updates. If you do not wish to update at that time, we will remove the outdated forms to ensure our clients have the most up-to-date forms.

If you select Email, Fax, Phone, or US Mail:
We will contact you via your chosen method every 30 days to ask for updates.

Due to compliance issues, even if there is only a small one-word change in your form, we still need you to send an updated file. We will not alter the form content after we have received a form.
